In 2020, Children’s Futures successfully implemented its Nurse-Family Partnership NFP program, serving the perinatal needs of 147 families. By partnering with expectant mothers and new parents pre-birth, NFP improve outcomes through home-based support from dedicated nurses.
Health Outcome Highlights:
Insurance Coverage: Almost all eligible participants were provided health insurance.
Prenatal Care: All clients were recommed for a medical home setup.
Early Prenatal Visits: Over 71 of participating mothers initiated prenatal care before their first trimester, while more than 95 underwent alcohol, tobacco, drug screenings as well as depression and domestic violence assessments.
Health Insurance and Medical Home for Children: Almost 92 of children had health insurance coverage and were enrolled in a medical home. Furthermore, over 95 of the children received timely immunizations, atted well-child check-ups, and underwent developmental assessments by age two.

In 2022, Children’s Futures received numerous referrals1,522 in totalfrom prenatal clinics, OB offices, walk-ins, and outreach initiatives. This year, CF supported various services for approximately 1,099 clients across its home visiting, case management, fatherhood, and Doula programs.
Community-Based Centers:
CF continues to enhance community health by operating two neighborhood centersone at Children’s Futures headquarters and the other in collaboration with Mercer Street Fris. This setup ensures that families have access to center-based services tlored to their needs.
Free Pregnancy Testing: Avlable for all women across Mercer County every Monday and Friday through appointment only, and Tuesday-Thursday without an appointment from 10:30 am – 4:00 pm.
Parent Education Classes: Offered by CF to educate parents on various topics including child development stages, discipline methods, emotional management, and effective parenting routines.
The case manager plays a critical role in:
Enrollment Recruitment: Assisting families with accessing essential services like insurance, prenatal care, WIC, nutrition education, breastfeeding support, safe sleep practices, smoking cessation, immunizations, developmental assessments, lead evaluations.
Postpartum Support: Offering check-ups and reproductive life planning guidance to new mothers.
Preventive Health Screenings: Identifying depression, ATOD use, domestic violence, and referring for appropriate interventions.
Resource Navigation: Helping fathers find resources to improve their quality of life.
CF's fatherhood program fosters a supportive community:
One-on-One Mentoring: Experienced mentors guide new fathers through the challenges of parenting by sharing insights, experiences, and advice.
Father’s Forum: A safe space for fathers to share experiences, find solutions together, build accountability, and learn new skills.
D.A.P. Dads as Parents Activities: Encouraging bonding time between fathers and children through shared activities that promote positive relationships.
Mental Health Services:
CF provides comprehensive therapy options:
Individual Therapy: 90-minute sessions for up to eight participants med at managing stressors.
Wellness Group Sessions: 90-minute gatherings designed to alleviate emotional issues, offering stress management techniques.
